Undergraduate Student Clinical
​Placement Process

Please check with your school representative if you have a question about COVID-19 vaccination documentation or requirements as they may vary amongst organizations. 

After your school has confirmed a clinical opporutinity for you, you will work with your school contact to complete the following requirements and submit all information to your school, not the healthcare facility.

Advocate Aurora Health - Nursing and Medical Assistants please go to Nursing & MA Clinical Placement. All other disciplines will follow FVHCA requirements. 

Step 1:  Health Requirements/Background Checks
 (Note:  school should have these  already, but verify with school contact that they are current):
  • Health Requirements 
  • Hepatitis B Documentation Form 
  • Background Check Information
  • Medical Influenza Exemption 
Step 2: Complete General Orientation Requirements before coming onsite, starting with the online modules below:  
​1. Student Orientation Professional Expectation Module
​2. Student Orientation HIPAA Privacy and Security Module
3.​ Student Orientation Infection Prevention
4. Safety in the Workplace
Step 3: Complete a Facility Specific Orientation: 
(If you are utilizing an organization not listed below, jump to Step #3.) 
  • Ministry Health Care / Affinity
  • Agnesian Healthcare​
  • ThedaCare (You will receive onsite orientation on day 1; there is no online orientation to complete ahead of time)
  • ​Evergreen Retirement Community (contact directly for information)
Step 4: Complete and Submit Forms to School:
Read, review, print off and sign Forms A, B, & C and turn them in to your school representative.  Your signature indicates acceptance of responsibility for information presented.  For electronic signature, use Internet Explorer as it will not work in Chrome.
Form A: Confidentiality Agreement

Form B: Module Completion

Form C: Healthcare Facility Specific Orientation Completion
Form C information: If you did an online facility specific orientation in Step #3, fill this out with the date you completed, and send to your school for record keeping.  If an online is not available, print, fill out and sign after you complete onsite orientation and department safety walk.  Then - send it to your school for record keeping.   We do audit that it was returned to school since it is a regulatory requirement.
Next steps:  After school submits the RIF, your student access will be set up and documentation done - an email will be sent out to the student, school rep, and onsite preceptor with further student information prior to clinical start date- at that time, the student is welcome to contact his/her onsite preceptor regarding a schedule of hours within the approved date range. Thank you!

Final Reminders
  • Dress accordingly.
  • Some facilities may assign a badge or student badge backer to you; if not, always wear your school student badge while on site. 
  • Remember to keep patients at the center of your experience and practice safely.
  • Ask questions if you are not familiar with something.
  • Seek out experiences as they come available.
  • Have fun! Learn a lot!
