Getting Started
​

School representatives - please check with site contact if you have a question about clinical placements due to COVID-19. 
Before a clinical placement request can be considered, a current legal affiliation agreement between the school and the healthcare organization must be in place.

Step 1:  10-12 weeks prior to placement need*, school representative must submit a Graduate Request form  on behalf of the student.  Please note that many organizations do not allow students to reach out to potential preceptors directly.
​*Some facilities may have different deadlines for NP/PA requests - check with facility contact directly regarding timelines.


Step 2:  If approved, school rep to work with student to complete the orientation steps  
listed further down on this page so that school can complete the RIF/excel form

Graduate Required Student/Faculty Information form
 and submit to the facility 6-8 weeks prior to clinical start date. 
Note - only send the RIF/excel form; all other documents must be housed with school and available upon request, but NOT necessary to email. Every section must be filled in completely prior to sending.

​Orientation


Once placement has been confirmed, ALL of the following requirements must be completed and turned in to your school representative as soon as possible.  If there is a delay with your school's ability to submit regulatory paperwork, your experience could be delayed or cancelled.  Your school generally has everything under Step #1 as part of your school requirements, but please verify with them that everything is current so they can turn it in on your behalf. 
Advocate Aurora Health - Nursing - RN to BSN.
MSD & DNP (projects) please go to Nursing & MA Clinical Placement. All other disciplines will follow FVHCA Requirements. 
Step 1:  FVHCA Health Requirements/Background Checks:     
  • Health Requirements   (Note that COVID-19 vaccination requirements vary, your school can email site contact for details if needed)
  • Hepatitis B Documentation Form 
  • Background Check Information
  • To be considered for a medical exception to the influenza vaccination only -  Medical Influenza Exemption ​ ​
Step 2: Complete Online Orientation Requirements:​

3. Student Orientation Infection Prevention
4. Safety in the Workplace

​Step 3: Complete Facility Specific Orientation:
(If you are utilizing an organization not listed below, jump to Step #4.) 
  • Ministry Health Care / Affinity
  • Agnesian Healthcare​
  • ThedaCare  (Note: your facility specific orientation will happen in person on your first day onsite; there is no online version to complete)  ​
  • ​Evergreen Retirement Community (contact Evergreen directly for information)
Step 4: Complete and Submit Forms to School:
You need to print off and sign Forms A, B, & C and turn them in to your school representative.  Your signature indicates acceptance of responsibility and accountability for information presented.  (Note - for electronic signature,  use Internet Explorer as it does NOT work in Chrome).

​Form B: Module Completion

Form C: Healthcare Facility Specific Orientation Completion
Form C information: ​If you did an online facility specific orientation in Step #3, fill this out with the date you completed it and send to your school for record keeping.  If an online orientation is not offered, print this out before you arrive on your first day of clinical, fill it out, and sign after you receive the onsite orientation and department safety walk.  When complete, you need to send it to your school for record keeping.   We do audit that it was returned to school since it is a regulatory requirement.
