Step 1: Complete General Orientation Modules
As a faculty member, you are expected to complete online general orientation modules, which are the same ones that students must complete.

Step 2: Complete Forms A, B and C and turn in to your school representative completing paperwork.
Form A: Confidentiality Agreement
Form B: Module Completion
Form C: Site Specific Orientation
*Form C is done on day 1 and is the onsite orientation/safety walk. These forms are given to the unit leadership team for documentation upon completion.
Step 3: If you are also the person who is requesting the placement (not someone else at your school), follow steps on the school coordinator tab.

Faculty/Instructor Responsibilities
As an instructor, you will more than likely assist three (3) varying types of students. Below, we have outlined the steps that must be taken to ensure the needs of each student type are met. Please follow the method that is appropriate for the type of student that you are working with.
Precepted Students:
If you are Faculty or an Instructor responsible for following up or facilitating precepted students, please note and / or ensure the following:
- A faculty / instructor does not need orientation to facility if you are just checking in with students occasionally.
- Any onsite faculty/instructor providing direct patient care must complete facility orientation - please talk with each site directly regarding requirements.
- Ensure student completes required orientation onsite.
- Ensure healthcare facilities have correct faculty/instructor contact information, including cell phone numbers.
- Ensure preceptors are assigned.
- Provide feedback of clinical site.
Group Clinicals:
If you are Faculty or an Instructor responsible for clinical groups, you will need to:
- Ensure the student is competent in the designated clinical area
- Ensure the student completes an annual orientation to the facility
- Online FVHCA orientation modules completed.
- Facility Orientation is required.

- ThedaCare (no online version available, reach out to school coordinator or department leader prior to clinical start date).

- Ensure patient safety
- Provide direct supervision of students
- Provide feedback of clinical site
