Undergraduate Clinical Placement Process

For Advocate Aurora Nursing/MA requests ONLY, click here. All other Advocate Aurora requests follow the process below.

For all other sites, follow these steps:
Step 1: Read Placement Request Form Instructions.
Step 2: Complete Placement Request Form:
Step 3: Email Completed Form to site contact: Click here for a listing of FVHCA sites and contact information.
Step 4: Healthcare site will contact school with an answer; if approval is granted, continue to next step.
Step 5: Read Required Information Form (RIF) Instructions:
Step 6: Work with student to complete (RIF) Form:
Step 7: Email Completed RIF in Excel format

For all other sites, follow these steps:
Step 1: Read Placement Request Form Instructions.
Step 2: Complete Placement Request Form:
- Requests should be submitted 10-12 weeks before clinical start date (no earlier, please).
- Requests are needed for any student time onsite, whether involving patient care or not, such as a project.
- Include as much information as possible, including specific campuses or units - it's helpful to provide first/second choices.
- Every effort will be made to respond within a 3-5 week period.

Step 4: Healthcare site will contact school with an answer; if approval is granted, continue to next step.
Step 5: Read Required Information Form (RIF) Instructions:
- Must be updated annually if repeat placement, and all requirements must be valid through end date of rotation.
- Last 4 of social security number or student ID is no longer required.
- For Date of Birth (DOB), year is not needed - only month/day for EMR access set up.
Step 6: Work with student to complete (RIF) Form:
- Students will review/access the requirements on the undergraduate student page and submit documents to school, if not already available to school.
- RIF should be sent 6 weeks prior to clinical start date, but no later than 4 weeks prior, or start date could be delayed.
- School signature/date attests to school having actual copies of all requirements, which could be requested by healthcare organization at any time.
Step 7: Email Completed RIF in Excel format
- Please follow directions carefully about what is required for each section/box on the form, or it will be returned as incomplete.
- Only send RIF - documents or signature pages proving completion of requirements are to remain at school for recordkeeping.
- Site will send email or appropriate communication prior to start date.
