Undergraduate Clinical Placement Process
Once you have identified the facility you wish to utilize, follow the steps below to make a request.
For COVID-19 related questions, reach out to the site contact for information as it might vary amongst organizations.
For Advocate Aurora locations for Nursing/MA requests ONLY, click here. All other Advocate Aurora requests follow the process below.
For Ascension Health Care/Affinity facility requests (ALL areas) - Click here to access form for Ascension Health Care/Affinity facilities only.
Step 1: Read Clinical Placement Request Form Instructions: Click here to download the instructions.
Step 2: Complete Clinical Placement Request Form

Step 3: Email Completed Form to site contact: Click here for a listing of FVHCA sites and contact information.
Step 4: Request is Approved or Not able to approve by healthcare organization contact.
Step 5: Read Required Student/Faculty Information Form (RIF) Instructions (Must be updated annually if repeat placement). *Instructions are currently being updated due to recent RIF updates, coming soon! Version below will still be helpful until update is made.

Step 6: Work with student to complete Required Student/Faculty Information Form (RIF): Click here for the form.

- Keep in mind; submission of the form is NOT a guarantee of placement.
- Requests should be submitted 10-12 weeks before clinical start date (no earlier, please).
- Every effort will be made to respond to the request within a 3-6 week period but may be longer depending on multiple factors.
- If you haven't heard back in a reasonable amount of time, please contact the facility representative directly.

Step 4: Request is Approved or Not able to approve by healthcare organization contact.
- If approved continue to step 5.

- Note: please follow directions carefully about what is required for each section/box on the form, or it will be returned as incomplete.
- Students will review/access the requirements on the undergraduate student page.
Step 7: Email Completed Form

- Note - facilities only need the RIF; all other documents must be housed with school and available upon request, but NOT necessary to email.
- Every section must be filled in - reach out to the site contact if you have questions.
- RIF should be sent approximately 4-6 weeks prior to clinical start date (unless you have been given a different timeline by your contact).
