Health Requirements

The following are Uniform Health Requirements for Students Utilizing FVHCA Member Healthcare Facilities:


Tuberculin Test
Hepatitis B Vaccination
MMR
Varicella Vaccination
Influenza Vaccination
Tdap
CPR
Give the date of the last TB skin test or IGRA (quantiferon gold test OR T-spot).
If greater than 12 months, a two step is required, quantiferon gold test OR T-spot

​For known positive TB skin test or quantiferon gold, evidence of a negative baseline chest x-ray at or within one year of starting their initial clinical experience is required AND an annual TB questionnaire.
FVHCA Hepatitis B Documentation Form
AND 1 of the following Documented:

Hepatitis B vaccination series (or series + Titer)

OR

Signed declination

OR
​

A positive Hepatitis B titer
Dates of 2 MMRs

OR
​

Positive rubella titer, rubeola titer and mumps titer
Two (2) Varicella Vaccination dates

OR
​

Positive Varicella titer
Prior to November 15th for Fall session/semester, and prior to any Winter/Spring session/semester. 

​For Clinicals beginning April 1st and ending prior to October 1st. Students are required to be compliant with facility policy.

​Check with your school coordinator for more information.

(Subject to CBC accommodations)
Documentation of one time dose of the Tdap vaccine and then Td every 10 years.
Current valid American Heart Association Healthcare Provider (BLS) documentation required. 

Note:
​
Not needed if not in direct patient care.

May or may not be required for CNA, check with School Coordinator or facility

PLEASE NOTE: Physical exams are not required, however, some non-FVHCA facilities may have more stringent requirements.  Please check with each facility for more information.

IMPORTANT:  Health care requirements are determined by the organization and the FVHCA and not by the specific department within an organization.  For example, all of Agnesian HealthCare clinical students are accountable for the above requirements and a specific department or unit cannot add or change these requirements.
