FVHCA has developed uniform Background Check requirements for all students utilizing FVHCA facilities. The following are a list of all checks required:
Required Annually and within 48 hours of any change in status:
1. Background Information Disclosure Form (BID): Click Here
Updated Every Four Years:
1. Department of Justice Criminal Background Check/Department of Health and Family Services Caregiver Background Check (DHFS): https://recordcheck.doj.wi.gov/
3. SAM (formerly EPLS) Check: https://www.sam.gov/portal/public/SAM
The above checks must be performed and cleared without criminal record. Any positive findings must be discussed with the clinical facility and permission granted for the student to proceed with clinical placement.
Regulatory approval, employment as a caregiver, and nonclient residency at and contracting with an entity are prohibited until rehabilitation approval is received, for all entities and programs that serve any clients who are under the age of 18. (For additional federal foster care bars, see part III. below.)
*NOTE: The following crimes require proof of rehabilitation in order to be eligible to work in healthcare in Wisconsin:

- Click on the "PUBLIC ACCESS" tab > Under Background Request: click on Submit Request > Security warning read and check the accept box, and continue > Background Request Type drop down arrow select "Caregiver" > Fill in the Required fills and click continue (Background check report is required)
