Welcome to the Fox Valley Health Care Alliance Graduate Student clinical placement web page.

Before being placed in a clinical setting, your school coordinator or faculty will provide you instructions on how to select a preceptor. 

School coordinators and/or faculty will submit placement requests using the instructions and form below.

Once you have been accepted at a facility and your placement has been confirmed, you must complete the following requirements BEFORE your clinical start date.

Step 1:         FVHCA Health Requirements and Background Checks must be completed (confirmed through your school)

Step 2:         Complete Orientation Training (2 .pdf presentations)

Step 3:         Complete Facility Orientation where applicable

Step 4:         Turn in completed forms from the two online learning modules to your school representative or faculty/instructor
